Ensuring that life-saving antimicrobials remain available as effective treatment options in the face of rapidly rising levels of antimicrobial resistance will require a massive and coordinated global effort. Setting a collective direction for progress is the first step towards aligning global efforts on AMR. This process would be greatly accelerated by adopting a unifying global target — a well-defined global target that unites all countries and sectors. The proposed pandemic instrument — with its focus on prevention, preparedness and response — (...) represents an ideal opportunity to develop and adopt a unifying global target that catalyzes global action on AMR. We propose three key characteristics of a unifying global target for AMR that — if embedded within the pandemic preparedness instrument — could rally public support, funding, and political commitment commensurate with the scale of the AMR challenge. Background The American Association of Medical Colleges has called for incorporation of the health humanities into medical education, and many medical schools now offer formal programs or content in this field. However, there is growing recognition among educators that we must expand beyond empathy and wellness and apply the health humanities to questions of social justice – that is, critical health humanities. In this paper we demonstrate how this burgeoning field offers us tools for integrating social justice into medical education, (...) utilizing the frameworks of critical consciousness and structural competency. Practice of health humanities Critical health humanities can be applied at multiple levels of learners, and in a variety of contexts. We are two physician-writers who have developed several educational programs that demonstrate this. We taught a seminar that introduced first-year and second-year undergraduates to concepts such as social determinants of health, intergenerational trauma, intersectionality, resilience, and cross-cultural care through works of fiction, poetry, film, podcasts, stand-up comedy, and more. Through creative projects and empathic reflection, students engaged with the complexities of structural forces that create and maintain health disparities. Medical students in their clinical years can engage in critical health humanities learning experiences as well. We teach several multidisciplinary electives that address social (in)justice in medicine, as well as mentor fourth-year students engaged in independent electives that foster critical awareness around health equity and ethics. Beyond the classroom, we have actively engaged in critical health humanities practices through story slams, literary journal clubs, conference presentations, and Grand Rounds. Through these activities we have included learners at GME and CME levels. These examples also demonstrate how community engagement and multidisciplinary partnerships can contribute to the practice of critical health humanities. Conclusion In this paper, we explore the growing field of critical health humanities and its potential for teaching health equity through narrative practices. We provide concrete examples of educational activities that incorporate critical consciousness and structural competency – frameworks we have found useful for conceptualizing critical health humanities as a pedagogical practice. We also discuss the strengths and challenges of this work and suggest future directions. (shrink)

In fungi, cell fusion between genetically unlike individuals triggers a cell death reaction known as the incompatibility reaction. In Podospora anserina, the genes controlling this process belong to a gene family encoding STAND proteins with an N‐terminal cell death effector domain, a central NACHT domain and a C‐terminal WD‐repeat domain. These incompatibility genes are extremely polymorphic, subject to positive Darwinian selection and display a remarkable genetic plasticity allowing for constant diversification of the WD‐repeat domain responsible for recognition of non‐self. Remarkably, (...) the architecture of these proteins is related to pathogen‐recognition receptors ensuring innate immunity in plants and animals. Here, we hypothesize that these P. anserina incompatibility genes could be components of a yet‐unidentified innate immune system of fungi. As already proposed in the case of plant hybrid necrosis or graft rejection in mammals, incompatibility could be a by‐product of pathogen‐driven divergence in host defense genes. (shrink)

Après avoir présenté les grandes lignes de la réflexion phénoménologique de Wittgenstein en 1929 et son abandon de Vidée d'un langage phénoménologique, je montre qu'il fait face à un dilemme à propos de la notion de règle. Si on regarde l'expression symbolique d'une règle sous le mode physicaliste, on ne peut pas voir le lien entre celle-ci et ses applications. Comment peut-elle, de par elle-même, servir de guide à nos actions ? Il faut une intention, une interprétation, ce qui fait (...) de la règle un objet phénoménologique. L'abandon du langage phénoménologique doit cependant forcer Wittgenstein à reconnaître qu'une règle en tant qu'objet phénoménologique ne peut pas être comparée à ou être utilisée pour déclencher une action qui se déroule dans le monde physique. Je montre que c'est en tentant de sortir de ce dilemme que Wittgenstein en viendra aux conceptions exposées dans ses remarques sur « suivre une règle ». J'examine en dernier lieu quelques conséquences de cette interprétation sur celle de la philosophie des mathématiques de Wittgenstein.After presenting a broad sketch of Wittgenstein's phenomenology; in 1929, and of his abandonment of the idea of a phenomenological language, it is shown that the notion of a rule left him with a dilemma, if the symbolical expression of a rule is understood in a physicalist mode, one cannot see the link between it and its applications. How could it, of itself guide our actions? One needs an intention or interpretation, thus turning the rule into a phenomeno/ogica I object. The abandonment ofthe p he nojne no logical language must have foiled Wittgenstein to recognize that a rule, qua phenomenological object, cannot be compared to or used to trigger an action in the physical world. It is then shown how Wittgenstein reached, while trying to solve this dilemma, the conception of rules presented in the remarks on 'following a rule". Lastly, some of the consequences of this new reading on the interpretation of Wittgenstein's philosophy of mathematics are examined. (shrink)

Russell claims that ordinary proper names are eccentric, i.e. that the semantic referent of a name is determined by the descriptive condition that the individual utterer of the name associates with the name. This is deeply puzzling, for the evidence that names are subject to interpersonal coordination seems irrefutable. One way of making sense of Russell’s view would be to claim that he has been systematically misinterpreted and did not, in fact, offer a semantic theory at all. Such a view (...) is put forward in Sainsbury Departing from Frege, Routledge, London, 2002). Sainsbury claims that Russellian descriptivism is not the theory that the thought in the mind of the speaker determines the semantic reference of a name, but simply a theory about the thought in the mind of the speaker using a name. I argue that the truth is subtly different, and points the way towards an intuitive explanation of Russell’s eccentricity. (shrink)

Biomedical research has a strong impact on a country’s scientific-technological and socioeconomic development. It can make a significant contribution at three different levels: promotion of public health; the exchange of knowledge within the scientific community; and economic/ financial profitability. Africa only attracts ~3.3% of the world’s clinical research. This small proportion is due to, among several factors, the absence of two fundamental aspects: specific robust legislation and capacity for regulatory and ethical evaluation. There are five Portuguese- speaking African countries – (...) Angola, Cabo Verde, Guinea-Bissau, Mozambique and São Tomé and Príncipe. Despite presenting different degrees of clinical research development, these countries show a common serious insufficiency in the minimum conditions necessary for hosting, collaborating or leading biomedical research projects. These countries are also excluded from the support of international organisations for capacity building because of the language barrier. The Biomedical Ethics and Regulatory Capacity Building Partnership for Portuguese-Speaking African Countries project has developed methods to build capacity and partnership through research on national law, comparisons with international standards and by issuing recommendations at a national level. Through collaborative intensive training, trainees are taught to become educators at a national level. This, in turn, creates a sustainable impact at country level. BERC-Luso is a pioneering project, owing to a combination of innovative features. The partnership project: was developed exclusively in Portuguese; involves a large number of African countries, and national and international institutions; has been further enhanced by the diversity and complementarity of its action plans; and has also benefited from a broad participatory methodology based on resolution of recipients’ problems by the recipients themselves. PITTION MY PURPOSE IN THIS PAPER is to discuss an aspect of Hume's reading of Pierre Bayle, the French "Philosopher of Rotterdam. ''1 I am not concerned here with the identification of Hume's direct borrowings from Bayle in the Treatise, nor with the much wider problem of a probable influence of Bayle on (...) Hume. It is indeed possible to identify in the Treatise a number of specific points taken from Bayle. Kemp Smith listed four at least: the account of skepticism, the discussion of Spinoza, the discussion of animal intelligence, and the criticism of the argument from design. 2 All of these come from Bayle's Dictionnaire. External evidence confirms that in 1732 Hume had a work of Bayle, presumably a copy of the Dictionnaire. Other borrowings from Bayle have been identified in the Treatise and in Hume's other works. This is no surprise, for in the eighteenth century Bayle's Dictionnaire was used as it was intended in the first instance, that is, as a reference work. In detailing the borrowings from Bayle, Kemp Smith also touched upon the broader question of influence when he stated that Hume's Pyrrhonism, particularly in relation to his treatment of religion, may also come from Bayle. Many recent scholars have followed suit. A problem of influence however is always a difficult one, and particularly so in this case. There was, as R. H. Popkin has pointed out, a broad awareness of continental skepticism among British philosophers of the eighteenth century. 3 However, apropos Berkeley, A. A. Luce has remarked that the trouble with eighteenth-century continental skepticism was that it had various objectives and took various forms. 4 This is true of Bayle himself. Within skepticism Bayle's position was known for its extremism, and it was this extremism that brought him notoriety. During the eighteenth century Bayle was the philosopher who promoted a systematic use of the Pyrrhonic mode of argument. He was acknowledged above all for having set up a double paradox: that any attempt at a rational theodicy must lead to Manicheism and that to argue for a rationalist Christian ethics is to argue the case of an atheistic one as well. It is doubtful whether many of those who do refer to Bayle during this period understood him beyond these terms, whether they sought to understand, or whether they even bothered to read Bayle's lengthy arguments. It is clear that few really grasped his complex, often ambiguous objectives. Mention of a Baylian position by an eighteenth-century writer is therefore no guarantee that the writer has actually read Bayle's works: Bayle is a sort of figurehead for Pyrrhonism, Manicheism, or often even atheism, but one can doubt that there is behind the mention of these basic positions any serious reflection on his thought. There are exceptions of course. Luce and H. M. Bracken have shown that Bayle played a seminal role in the formation of Berkeley's thought? But as a rule, knowledge of Bayle among writers of the Enlightenment was vague; references to him, nonspecific. In most cases it is scarcely possible to speak of an influence. My aim in this... Despite Vancil’s (1979) proclamation over twenty years ago that topoi have been abandoned in argument theory, this essay contends that topoi should have a vital role in contemporary argumentation theory. Four key areas are identified where topoi are (or can be) essential tools for argumentation: Locating argument, building argument, development of critical thinking, and argument pedagogy. As a result, teachers and students of argument can both benefit from a (re)discovery of topoi.
